Blüm Health, LLC is a limited liability corporation providing culturally responsive pediatric mental health, autism services, and primary care to communities in Hawai'i.

Description of Services

Blüm Health, LLC provides healthcare services including but not limited to pediatric mental health assessment and treatment; autism spectrum disorder evaluation and intervention services; primary care services for pediatrics and families; family therapy and support; and related healthcare coordination services.

The information provided on our Site is for general informational purposes only and does not constitute medical advice. Always seek the advice of a qualified healthcare provider with any questions you may have regarding a medical condition.

User Responsibilities

By using our Site and services, you agree to:

By checking an SMS consent checkbox and providing your mobile phone number, you expressly agree to receive SMS messages from Blüm Health, LLC at the phone number provided. Message frequency varies depending on your interaction with us, your appointments, and the services you request. Message and data rates may apply.

• Types of messages: Messages may include responses to inquiries, appointment reminders, scheduling updates, care coordination messages, clinic updates, and, where separately consented to, product, service, wellness, or promotional updates.
• Opt out: Reply STOP to cancel or unsubscribe from SMS messages.
• Help: Reply HELP for assistance, call (808) 862-8795, or email info@blumhealthcare.com.
• Consent: Consent to receive SMS messages is not a condition of receiving healthcare services or making a purchase.
• Carriers: Wireless carriers are not liable for delayed or undelivered messages.

Healthcare Services

Our healthcare services are provided by licensed healthcare professionals. By engaging our services, you acknowledge that healthcare services require your active participation; treatment outcomes cannot be guaranteed; you have the right to refuse treatment; and you are responsible for providing accurate health information.

Telehealth services, where offered, are subject to additional terms and may have limitations compared to in-person care. Telehealth services may not be appropriate for all conditions or situations.

Appointments and Cancellations

We request that you provide at least 24 hours' notice for appointment cancellations or rescheduling. Repeated no-shows or late cancellations may result in cancellation fees or discharge from services. We understand that emergencies occur and will consider individual circumstances.

Payment and Insurance

Payment is due at the time of service unless other arrangements have been made. We accept various forms of payment and participate with select insurance plans. You are responsible for understanding your insurance benefits and for any amounts not covered by insurance.

Financial assistance may be available for qualifying patients. Please contact our office for more information about our sliding fee scale and financial assistance programs.

Intellectual Property

All content on our Site, including text, graphics, logos, images, and software, is the property of Blüm Health, LLC or its content suppliers and is protected by copyright and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content without our prior written consent.

Disclaimer of Warranties

Our Site and services are provided "as is" and "as available" without warranties of any kind, either express or implied. We do not warrant that our Site will be uninterrupted, error-free, or free of viruses or other harmful components.

These Terms shall be governed by and construed in accordance with the laws of the State of Hawai'i, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be resolved in the courts of the State of Hawai'i.

We reserve the right to modify these Terms at any time. We will notify you of material changes by posting the updated Terms on our Site with a new effective date. Your continued use of our Site after such changes constitutes acceptance of the modified Terms.
